PRIVACY STATEMENT

Valley Anesthesia Associates is a group of Physician Anesthesiologists (MDs) and Certified Registered Nurse Anesthetists (CRNAs) who work as a team to provide a wide variety of anesthesia services at Valley Medical Center. Valley Anesthesia Associates is committed to protecting our patients' privacy, and we work cooperatively with Valley Medical Center to comply with the Health Insurance Portability and Accountability Act of 1996 ("HIPAA") Privacy Regulations. Upon admission as a patient to Valley Medical Center, you will receive a copy of the Valley Medical Center Notice of Privacy Practices, which will describe how personal information about you may be used or disclosed in order to provide you with health care services, inform you of your rights regarding your personal health information, and advise you how you can get access to that information. Prior to hospital admission, you may also obtain a paper copy of the Notice of Privacy Practices from any Valley Medical Center Clinic or at the hospital's Main Admitting Department.
